IME/Impairment Rating Policy
All IME’s are scheduled with our Medical Director, Howard Konowitz, MD
All patients will receive a New Patient Packet with an Initial Pain Questionnaire and Scantron Packet. The patient must arrive for their appointment with all Questionnaires completed.
A patient who doesn’t receive a New Patient Packet prior to the appointment must arrive (1) one hour before their scheduled appointment to complete the paperwork.
If the patient refuses to fill out the Initial Pain Questionnaire, the physician will not be able to perform an examination and the visit will be terminated.
If the patient does not keep their scheduled appointment without proper notification, or the visit has to be terminated due to the refusal or untimely completion of the Initial Pain Questionnaires, a fee will be billed to the party who scheduled the IME visit.
