IME/Impairment Rating Policy

Appointments

  • All IME’s are scheduled with our Medical Director, Howard Konowitz, MD

  • All patients will receive a New Patient Packet with an Initial Pain Questionnaire and Scantron Packet. The patient must arrive for their appointment with all Questionnaires completed.

  • A patient who doesn’t receive a New Patient Packet prior to the appointment must arrive (1) one hour before their scheduled appointment to complete the paperwork.

  • If the patient refuses to fill out the Initial Pain Questionnaire, the physician will not be able to perform an examination and the visit will be terminated.

  • If the patient does not keep their scheduled appointment without proper notification, or the visit has to be terminated due to the refusal or untimely completion of the Initial Pain Questionnaires, a fee will be billed to the party who scheduled the IME visit.

Medical Records For Appointment

  • All medical data needed for the IME must be received in our office 7 days before visit. If a special exception is needed simply call Tamara @ 847-470-8740. We will be happy to assist you.

  • Medical data needed for the IME can be uploaded at the time of your schedule request. The upload option appears on the last page of the IME Request form.

  • Medical data needed for the IME appointment can ALSO be uploaded using the Secure File Upload Form. An appointment date and case number are required to use this function.

  • Medical data needed for the IME can also be faxed to 847-470-8750

  • Medical data needed for the IME can also be sent by US Mail to:

  • Please do not mail or fax medical data to any other office or fax number. This may result in the IME physician not receiving critical information to complete your IME request, or possible cancellation of the IME. Only send medical data to fax number and/or address listed above.

Imaging For Appointments

  • Please do not mail X-ray or imaging films to our office. Please have the patient bring them to the appointment.
